Main Registration Form
Music questionnaire and personal information form. (due 2 months before arrival date)
This will give us an idea of where you're at musically and what your goals for the camp are. Express yourself freely, there are no right or wrong answers, the more we know about you in advance the more we can be prepared to meet your needs.
The forms below need to be filled out for all minors attending summer programs.
Primary information and health form. (due 2 months before arrival date)
This form needs to be filled out completely by your parents and family doctor. Schedule a check up immediately because this form needs to be signed by your doctor.
Medication form (due 2 months before arrival date)
This form needs to be filled out by your parents and your doctor if you will be taking prescription medication during the camp.
Release and potluck form. (due upon arrival at camp)
This form will tell us who is authorized to pick you up on the last day of the camp and how many people your family will be bringing to the show and potluck afterwards (as well as what sorts of food/drink your family will add to the feast!)
